Before any major treatment can be started on a sick or injured pet, we must have the signature of the financially responsible person on file. This means that when you arrive at the SAEC with your pet, you must fill out our Financial Policy and Treatment Authorization form before we bring your pet back into an exam room or the treatment area. (If necessary, we will provide life-saving treatment–for example, oxygen, pressure on a puncture wound, a quick initial assessment, etc.–before the form is signed.)

If your pet is in serious condition and you are worried about taking the time to fill out the Financial Policy and Treatment Authorization form once you arrive at the clinic, you can click on the link below, print the form, and fill it out beforehand so that we can start treatment on your pet immediately.

Any time a patient will be staying at our facility for hospitalization or to have a procedure performed, we must know the owner’s wishes regarding resuscitation in the event that the pet stops breathing or his heart stops. You may elect either CPR (CardioPulmonary Resuscitation) or DNR (Do Not Resuscitate).

Most of us would love for our pets to live forever, but the sad reality is that CPR rarely succeeds, either in animals or in humans. When resuscitation is successful, the patient often suffers impaired mental function for the rest of his life; this happens because of the loss of oxygen to the brain during the time the patient was not breathing or his heart was stopped.
